Visceral fat is the adipose tissue packed around the liver, pancreas and intestines. It matters more than the fat you can pinch because it is metabolically active โ it releases inflammatory compounds into the portal circulation, and it is associated with type 2 diabetes and cardiovascular disease independently of total body weight.
Which raises an awkward question: how do you measure something you cannot see, without a hospital?
#### The honest hierarchy of methods
**CT and MRI are the gold standard.** They image the abdomen directly and quantify visceral adipose tissue in square centimetres. They are also expensive, involve either radiation or a long scan, and no one is getting one to track progress.
**DEXA** is the common clinical compromise. It estimates visceral fat from a whole-body scan and correlates well with CT, but it still requires a clinic visit.
**Bioimpedance scales** โ the bathroom scales that display a “visceral fat rating” of 1 to 59 โ deserve more scepticism than they get. They pass a small current through the body and infer composition from resistance. The reading is highly sensitive to hydration, recent meals, and skin temperature, and the visceral figure specifically is derived from a proprietary model the manufacturer does not publish. Two scales will disagree with each other, and the same scale will disagree with itself before and after a glass of water.
**Tape measure and a wall.** This is the one that survives contact with reality, and the evidence behind it is better than its simplicity suggests.
#### Why waist-to-height ratio holds up
Waist-to-height ratio (WHtR) is your waist circumference divided by your height, in the same units. That is the entire calculation.
Browning, Hsieh and Ashwell reviewed 78 studies for *Nutrition Research Reviews* in 2010 and found WHtR predicted cardiometabolic outcomes better than BMI โ mean area under the ROC curve of 0.704 for WHtR against 0.671 for BMI. More usefully, the boundary value that emerged from studies across fourteen countries and multiple ethnic groups converged on the same number for men and women: **0.50**.
A 2012 meta-analysis by Ashwell, Gunn and Gibson in *Obesity Reviews* went further, finding WHtR superior to waist circumference alone as well as to BMI for screening adult cardiometabolic risk.
This is now embedded in clinical guidance. NICE guideline NG246, published in January 2025, uses WHtR to classify central adiposity: below 0.5 is healthy, 0.5 to 0.59 indicates increased central adiposity, and 0.6 or above indicates high central adiposity.
#### The rule of thumb that actually works
Keep your waist to less than half your height.
That is the whole 0.5 threshold, restated. A person 175 cm tall wants a waist under 87.5 cm. It requires no calculator, no chart, and no conversion between unit systems โ the ratio is dimensionless, so it works identically in centimetres or inches.
#### How to measure the waist properly
The number is only as good as the measurement, and this is where most people introduce more error than the method itself contains.
1. **Measure against skin**, first thing in the morning, before eating.
2. **Find the midpoint** between the bottom of your lowest rib and the top of your hip bone. This is not your trouser waist, which sits lower and will read smaller.
3. **Breathe out normally** and measure at the end of a relaxed exhale. Do not suck in, and do not hold a deep breath.
4. **Keep the tape horizontal** and snug without compressing the skin.
5. **Measure twice.** If the two readings differ by more than a centimetre, do it again.
#### Where age and sex come in
WHtR does the heavy lifting, but two things modify what a given ratio implies. Visceral fat accumulates with age, particularly after 40, as fat redistributes from subcutaneous to visceral depots even when total weight is stable. And at the same WHtR, men typically carry more visceral fat than premenopausal women, who preferentially store subcutaneously.

It is a heuristic, not a clinical measurement, and it says so on the page along with the research its thresholds come from.
#### What to do with a number you do not like
Visceral fat responds well, and responds early. Aerobic exercise at 150 to 300 minutes per week of moderate intensity produces measurable reductions over 8 to 12 weeks. Because visceral fat is mobilised before subcutaneous fat, waist measurements often improve before the scale moves at all โ which is precisely why tracking waist rather than weight keeps people going through the period when the scale is being unhelpful.
Measure monthly, not daily. Day-to-day variation in a waist measurement is mostly gut contents and hydration, and reading meaning into it will only frustrate you.
