Why Your BMI is Normal but Your Body Fat is High

BMI is a population-level tool, not a diagnostic one. A person can have a “Normal” BMI (18.5–24.9) yet possess high levels of internal fatβ€”a condition clinically known as **Normal Weight Obesity** or **TOFI** (Thin on the Outside, Fat on the Inside).

#### The Danger of Visceral Fat
Unlike subcutaneous fat (the kind you can pinch), visceral fat acts as an active endocrine organ. It secretes inflammatory cytokines directly into the portal vein, reaching the liver and interfering with insulin signaling. This is why TOFI individuals often have blood markers (glucose, lipids) similar to those with clinical obesity.

#### The Data-Driven Solution
1. **The WHtR Metric:** Research in *The Lancet* suggests that **Waist-to-Height Ratio (WHtR)** is a better predictor of lifespan than BMI. Aim for a WHtR of <0.50. Check yours now with the [Visceral Fat Estimator](/visceral-fat-calculator/), which uses WHtR as its primary driver. 2. **The Anabolic Push:** Resistance training is non-negotiable. Muscle is metabolically active tissue that improves glucose disposal. 3. **Protein Leverage:** The body will continue to signal hunger until protein needs are met. Aim for 1.6g to 2.2g of protein per kg of body weight to shift your composition. **Reference:** *Ashwell, M., & Gibson, S. (2016). Waist-to-height ratio as an indicator of early health risk.
